Dust inside a lens is not that unusual, nor fungus unfortunately. This story from Roger Cicala and co at LensRentals demonstrates that these problems* are not all we have to worry about, although it does appear to be a very rare occurrence.

* Dust inside a lens is not actually an issue and as this story shows, even quite a large object inside a lens does not impair optical performance. Dust on the sensor is actually a more significant problem.
